Follow-Up Care

AfterOurs refers patients back to their primary care provider for follow-up care. When a patient has not designated a primary care physician, they are referred to a trusted community physician, so they can begin developing the long term primary care relationship that is in their best healthcare interest. We provide your physician with a summary of the care you've received, so they may arrange appropriate follow-up care for you. At your physician's request, we can perform follow-up procedures (e.g., removal of stitches).

Lab Tests and X-Rays

When lab results are not immediately available, AfterOurs will contact both you and your designated primary care provider with your results. X-rays are immediately reviewed by our staff physician and if necessary, forwarded to a radiologist for overread. X-ray results will be shared with you and your family physician. 

Moving to a hospital or other medical facility

If you need hospitalization, we will make all arrangements for your admission to the hospital of your choice. We will forward a copy of your treatment records to the hospital, and notify your primary care physician of your visit. While moving patients to another facility is a rare occurance, our goal is to make this transition as smooth as possible.
