The sports physical consists of a series of steps. The first step is obtaining a thorough medical history. There are some underlying disabilities that preclude people from specific sports. Cardiac issues and history of concussions are two common examples. There are other medical conditions that may require additional monitoring during physical activity. Diabetes is a good example of this.
Next, the medical exam evaluates whether person is physically capable of performing the duties of the activity. The exam will typically consist of heart and lung evaluations, neurological evaluations, and musculoskeletal evaluations.
Other exams may include blood pressure and pulse, testing vision and hearing, and checking eyes and ears. In some cases, an electrocardiogram (ECG) test may be performed to check for heart issues.
The sports physical will usually end with the medical practitioner addressing any outstanding issues and answering any questions. After this, the practitioner will write a report detailing whether participation in the activity is recommended.

Types of physicals
More extensive sports physicals for specialty sports or institutions may also include these additional physical evaluations:
- Nutritional evaluation and assessment: Nutritional status will be evaluated including testing for allergies and eating disorders.
- Drug tests: Participants may be screened for any steroidal or illicit drug abuse. Blood and urine tests are taken.
- Mental health assessment: Mental well-being is important to create a healthy sports environment
- Physical stress tests: Includes many physical activities such as running miles, jumping, rock climbing, push-ups, and pull-ups to test physical strength.
